Contact & Scheduling Instructions

Thank you for your interest in scheduling with us. If you are having any trouble with online scheduling or prefer to schedule by phone, please feel free to contact us at 206-546-6000. We also offer free 10 min consults with Dr. Hancock to determine if she is the right fit. Chelsea, Dr. Hancock’s office assistant is available to help by phone Monday-Friday from 9-1 PST.

Please note, per licensure limitations, we can only accept patients who reside in Washington state.

Existing patients: Please log into your ChARM account and use the “Request an Appointment” feature to schedule.

New Patients:

1. Request a a new patient appointment below. If you do not see the appropriate appointment type, please hover your mouse over the options and scroll.

2. You will be directed to a pre-screening form, which we will review before approving your appointment. If needed, we may request a quick consult to determine if our practice is the best fit for you.

3. We require a card on file to reserve your appointment time. Please be sure to review our cancellation policy in the patient policy information provided. We will charge card on file for any no-shows or for cancellations made with less than 2 business days notice.

4. Once your appointment is approved you will receive an invitation to set up your ChARM account with instructions on how to complete paperwork.

   ***Please note we are in network with the following insurance plans: Regence, Premera, Lifewise, and First Choice. We are also in network with Aetna and United Healthcare but are limiting new patients. We are out of network with Cigna and are happy to bill out of network.
